Short Description
Ability to logically and independently plan and organize work assignments. Excellent communication and interpersonal skills. Should possess the initiative and ability to set and achieve high standards of performance. Ability to make progress on multiple assignments under time constraints. Must possess analytical, problem-solving, critical thinking, and decision-making skills. Ability to express ideas or make recommendations concerning job related and in writing. Ability to conduct analyses and generate reports to reflect findings. Ability to learn specific job duties and complete detailed work assignments. Ability to learn and maintain knowledge of basic concepts and techniques. Ability to perform the manipulation of numbers and basic math calculations.
Experience Required
- Completes mandatory components for pulmonary rehabilitation patients: physician prescribed exercise, education or training, psychosocial assessment, outcome assessment and individualized treatment plan (ITP) initial and every 30 days. ITP specifically individual exercise programs in accordance with CMS/AACVPR/AHA/ACSM guidelines under the direction of a licensed physician.
- Completes initial assessment of outpatients according to department standards. Assesses patient’s risk factors as well as co-morbidities. Anticipates patient needs and consistently addresses patient needs and concerns. Evaluates and reassesses interventions over time to assure best outcomes for the patients.
- Leads participants in prescribed exercise therapy. Plans for the patient discharge by working with the health care team to ensure timely discharge. Instructs patients in exercise principles and outlines guidelines for home exercise program.
Completes initial respiratory assessment data including biophysical, psychosocial, environmental, self care, discharge planning factors with experimental knowledge to refine individualized treatment plan.
Consistently documents patient evaluation, departmental data, goals, daily patient notes, individual treatment plan, outcomes, and patient exercise sessions in a timely manner with a high degree of accuracy.
Education Requirements
- A bachelor’s degree in Exercise Science is required, a master’s degree preferred, issued by an accredited university or college.
License/Certification Requirements
- ACSM/NSCA Certification is preferred.
- Current CPR/ BLS required.
- ACLS/Rhythm course highly recommended.
Special Requirements
- Required: Minimum of 3-5 years’ experience in Cardiopulmonary Rehabilitation.
- Must have experience in exercise prescription, education, counseling participants with chronic cardiopulmonary disease and individuals with risk factors
The New Jersey Pay Transparency Act requires disclosure of the pay range for this position.
A salary offer will vary based on the job role, candidate experience, qualifications, internal pay equity and market data.
