Short Description
The Community Outreach Coordinator is responsible for coordination of and deliver/pick-up to and from all community school and hospital partners. as well as run all outreach/awareness programs with these community members. This position is expected to maintain the highest level of hospitality and customer service in a manner consistent with policies, guidelines and the Ronald McDonald House mission and Cooper University Health Care.
Experience Required
3 - 5 years experience in community based project planning, public health programming, case management, working with grant funded programs, or related work.
Ability to manage a variety of stakeholders, provide direct patient services, conduct community outreach, and deliver education to EMS personnel and/or Skilled Nursing Facility personnel
Experience in community outreach/education programs.
Develop and implement a strategic outreach plan to engage target populations and promote program awareness.
Schedule, attend, and actively participate in community outreach events, health screenings, and educational programs.
Engage directly with patients on issues related to social determinants of health and connect them to appropriate services and resources.
Collaborate effectively with clinicians, administrators, patients, and community organizations to build and sustain partnerships.
Meet patients in community settings to reduce barriers to care and improve access to services.
Liaise with emergency services and/or skilled nursing facility personnel to enhance program visibility and improve patient outcomes.
Maintain up-to-date knowledge of community resources to support patient stability and recovery.
Educate emergency services personnel and community members about the BupeFirst program and its benefits.
Educate skilled nursing facility personnel and community members about the Center for Healing program and its benefits.
Document all patient interactions in the electronic health record and other systems for grant reporting and program tracking.
Work a flexible schedule and maintain reliable transportation to support outreach activities across various locations.
Ensure alignment with grant-funded program goals by tracking and reporting outreach activities, patient outcomes, and service referrals to meet deliverables and support continuous improvement and compliance.
Additional duties as assigned.
Education Requirements
Bachelor Degree in Public Health, Social Work, or related field required; Master’s Degree in Public Health, Social Work, or related field preferred
Special Requirements
Excellent communication skills, both written and verbal.
Computer literate, comfortable with Microsoft Office software (Outlook, Excel, Powerpoint, Word).
Electronic Health Record experience helpful
Demonstrated experience in patient-facing roles and community outreach, with the ability to engage diverse populations and foster trust through effective communication and culturally competent service delivery.
The New Jersey Pay Transparency Act requires disclosure of the pay range for this position.
A salary offer will vary based on the job role, candidate experience, qualifications, internal pay equity and market data.
