Short Description:

The Post Acute Care Nurse Navigator establishes regular contact with the facility care teams for Cooper University Health Care’s Preferred provider network to collaborate on the clinical assessements and clinical care plan. The navigator is responsible to assess and evaluate clinical services at SNF/LTC Facilities in addition to home care agencies within the preferred network to identify area as for clinical/process improvement (CARE PATHWAY integration/utilization). They shall provide recommendations for training and continuing education.
The RN Navigator will assess patients within SNF facilities and home care agencies weekly to ensure the progress of patient, and facilitate solutions to appropriate timely discharge and prevent acute readmission. They will collaborate with the preferred facility staff to promote patient self care management. The navigator will be responsible to coordinate data collection and performance improvement plans for the facility scorecards. Then ensure performance plans are implemented.

Special Requirements:

Scheduled Days / Hours: M-F 40hrs/week; License (or Certification):NJ-RN Required; Education:BACHELOR Required


Bachelor’s Degree required, Masters Preferred


Five (5) years experience in an acute/subacute rehab or long term care facility. Experience with case management or nursing leadership within these facilities is required. Experience with advanced care planning is preferred

Required License/Certifications/ Registration:

Current NJ-RN License (Registered Nurse)

Other Qualifications:

Excellent written /verbal communication and organizational skills
