Short Description
Supports, educates, and assists patients and their families, serving as a link to their doctors and supportive services. This position’s role is key in the implementation of multi-disciplinary care.
In conjunction with the multidisciplinary team, the Nurse Navigator is responsible for the coordination and management of patient journey through behavioral health care at Cooper Behavioral Health. The position serves a number of vital roles for the department and health system; first line-liaison between the patient and clinicians, as an educator for patients about their diagnosis, treatments, and potential side effects of said treatments. The nurse navigator also ensures patients are placed in clinically appropriate level of care via established algorithms and protocols, coordinates the injection clinic, and collaborates with social work to provide resources for patient seeking reliable information.
Education Requirements
BSN required, MSN preferred,
License/Certification Requirements
Current NJ-RN License or if working in an office in another state, that applicable state liscnese
Special Requirements
Scheduled Days / Hours: Monday-Friday 8:30-5; License (or Certification):NJ-RN Required; Education:BSN Required
