Short Description:


An advanced-practice prepared Registered Nurse who has completed educational preparation in Wound Ostomy and Continence Nursing and demonstrates professional competence in this specialized nursing field.

Demonstrates competence and knowledge in assessment, diagnosis, planning, implementation and evaluation of care for patients of all ages with WOC needs. Serve as consultant to all disciplines and departments, area of expertise, and patient population and participate on relevant committees and task forces as a clinical expert.

Organize, lead, and mentor clinical research and Evidence Based Practice (EBP) activities/projects of nurses and promote, as well as embed evidence- based practice into nursing and medical care. Support clinical nurses and medical team in translating research into practice. Present research and best practices at local, regional, and national conferences. Publish research findings as well as successes in improving standards of practice

Provides consultation for wound, ostomy, and continence using a multidisciplinary approach to treatment across a continuum of care and in a number of settings with direct recommendations to providers for treatments.

Improve practices and patient outcomes through collaboration on design, implementation, and evaluation of staff education and competencies, including drills, learning labs, and introduction of best practices

Advocate for best practices through the improvement of patient outcomes by advocating for individuals as well as populations within sphere of influence and area of expertise including the selction of appropriate WOC related treatments, technology (ex.Wound Vac) and products in collaboration with surgical specialties and value analysis.

TheCWOCN – Certified Wound Ostomy Continence Nurse scope of practice includes:

Preventing skin breakdown/Addressing and preventing peristomal skin complications

Treating and managing vascular ulcers, pressure ulcers and diabetic ulcers

Providing comprehensive wound management, including reducing causative factors, controlling odor, assessing nutritional needs, and providing and applying topical therapies

Assessing and treating draining wounds and fistulae

Designing pouch systems for draining wounds and fistulae

Implementing preventative actions to reduce the incidences of pressure ulcers and vascular ulcers

Providing instruction on proper wound dressing

Educating the patient, the patient’s family, and other support persons on ulcer prevention and treatment

Providing self-care, diet, and lifestyle education and support

Preventing peristomal/stomal complications

Providing rehabilitative care

Assessing stoma site marking so as to decrease long-term complications and encourage pouching system adherence

Reviewing and modifying pouch system problems

Special Requirements:


Schedule Days & Hours: Full Time (40 hours weekly); Monday - Friday; on call requirements

Education:

Advanced Practice Nurse, required.

Graduate of an accredited Wound Ostomy Continence Program required.

Board certified in Wound Ostomy Continence Nursing specialty.

Experience:

Membership in the national organization, WOCN.

A minimum of 5 years acute care nursing experience, 2 years of which is within the area of specialty practice.

Required License/Certifications/ Registration:

Current NJ-RN License (Registered Nurse)

Obtain and maintain certification by the New Jersey State Board of Nursing as an Advanced Practice Nurse (APN)

Takes and/or maintains certification in area of specialty, including, ACLS, NRP, BLS.

Prescriptive practice based on the development of written joint protocols with collaborating physician.

Receives approval from the Nurse Credentialing Committee.

Joint protocols are reviewed, updated and signed yearly

Other Qualifications:

Demonstrates effective written and oral communication skills and interpersonal skills.

Ability to articulate a vision and lead others through change.

Collaborative clinician capable of advancing programs.

Well-developed organizational, analytical and negotiating skills.
