Responsibilities

Short Description:


Provide initial comprehensive assessment including need/barrier identification of all patients.

Participate in comprehensive clinical and education programs including pre and post-visit care.

Implementation of group visit model and replication of best practices in other offices

Identify medical and social needs and barriers for at-risk individuals.

Develop and lead patient education for adult medical and surgical patients.

Follow up with patients on an ongoing basis to prevent hospital readmissions.

Coach and mentor LPNs.

Participate in team meetings to address progress and barriers in patient care.

Forge partnerships with local service organizations to improve care coordination of patients.

Other duties and responsibilities as directed.

Special Requirements:


Scheduled Days / Hours: Modnay-Friday; full time (40 hours weekly)

Education:

BSN and MSN required.

Experience:

13 years experience providing clinical services preferred, will also consider recent graduates

Experience in community/outpatient setting preferred

Required License/Certifications/ Registration:

Current NJ-RN License (Registered Nurse)

Apply for and/or maintain certification by the New Jersey State Board for Nursing as an Advanced Practice Nurse.

current BLS certification.

Other Qualifications:

Excellent organizational and interpersonal skills; strong oral/written communication skills is a must;

Demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences;

Ability to work collaboratively in a team and manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment;

Demonstrates ability to act independently;

Ability to work in multiple computer systems;

Bilingual English/Spanish preferred (not required)
